ABSTRACT

La rinoplastia es uno de los procedimientos quirúrgicos estéticos más complejos, no solo por las estructuras que se manejan para conseguir un resultado (mucosa, cartílago, músculo, hueso, tejido celular subcutáneo y piel) sino por las características propias de los pacientes que buscan una cirugía estética. Además, casi siempre hay un compromiso funcional, que se debe tratar de manera simultánea. El objetivo de este artículo es determinar cuáles son los factores más importantes que, manejados adecuadamente, se traducen en un desenlace exitoso. Para su mejor comprensión se consideran varios pasos: consulta prequirúrgica, diagnóstico, planeación de la cirugía, acto quirúrgico, manejo del postoperatorio y análisis de los resultados. The rhinoplasty is one of the most complex surgical aesthetic procedure, not only for the structures that they handle to obtain a result (mucous, cartilage, muscle, bone, cellular subcutaneous tissue and skin) but for the own characteristics of the patients who look for a cosmetic surgery. This without telling that almost always there is a functional commitment that must be handled simultaneously. The objective of this articulate is to determine which are the important factors that handled adequately are translated in a successful surgery. This divided in several passages like: consults presurgical, diagnose, planning of the surgery, surgical act, handling of the pos operating and analysis of the results. ABSTRACT

The nose muscles have a rudimentary action in humans with the exception of dilator naris muscle and depressor septi nasi muscle. The depressor septi nasi muscle has an important role pulling the nasal tip caudally while smiling, along with the orbicularis oris muscle. The detachment of this muscle from the premaxilla produces a dynamic rhinoplasty of the nasal tip. The objective of this study is to determine the importance of the detachment of the depressor septi nasi muscle and orbicularis oris muscle from the premaxillar bone in rhinoplasties of patients with a drooping nasal tip while smiling. 45 patients that underwent dynamic rhinoplasty were analized. All patients had an excellent postoperative evolution, with improvement of the drooping tip; this was demonstrated by direct nasolabial angle measuring and by photographic analysis. The detachment of the depressor septi nasi muscle and orbicularis oris muscle isa simple and reproducible technique, with limited morbidity and used during rhinoplasty that enhances the nasal tip rotation.

ABSTRACT

O tratamento das fissuras lábio-palatinas evoluiu muito nas últimas décadas. O tratamento atual da deformidade inclui técnicas ortopédicas de modelagem nasoalveolar como a Modelagem Nasoalveolar Passiva (MNA), que tem por objetivo a redução da largura da fissura alveolar e do palato, bem como da modelagem da região nasal afetada, logo após o nascimento. A MNA é iniciada na primeira semana de vida e se continua durante os primeiros meses, até a data do fechamento cirúrgico do lábio. O presente artigo relata o caso de um recém-nascido com diagnóstico ultrassonográfico de fissura lábio-alveolar. O diagnóstico precoce possibilitou o encaminhamento para o especialista, de tal forma que o tratamento foi iniciado logo após o nascimento. Ao final da MNA, a operação de correção da fissura nasoalveolar determinou o fechamento completo do defeito, e o aspecto da região tratada se assemelhava ao do lado não afetado.


The care of lip and palate clefts has progressed a lot in the last decades. Current treatment of the deformity includes orthopedic techniques of nasoalveolar molding such as the passive Nasoalveolar Molding (NAM), which is intended to reduce the width of the alveolar and palate cleft, as well as reshaping the affected nasal area, soon after birth. NAM is indicated to be performed in the first week of life and be continued over the next months, up to the eventual surgical closure of the lip. The present paper reports the case of a newborn with an ultrasonographic diagnosis of cleft alveolus and lip. Early diagnosis made it possible to promptly refer the patient to a specialist, so that treatment began soon after birth. At the end of NAM, the procedure to correct the nasoalveolar cleft determined the complete closure of the defect, and the aspect of the treated area can be appreciated.

ABSTRACT

Introdução: Nas rinoplastias, como em outras cirurgias, uma exposição adequada das estruturas manipuladas é fundamental para um bom resultado cirúrgico. Diversas são as técnicas empregadas, e essas variam, principalmente, em razão das alterações anatômicas encontradas. Objetivo: Avaliar quais as técnicas e manobras cirúrgicas mais realizadas em nosso serviço. Método: Análise retrospectiva das descrições de cirurgia dos pacientes submetidos à rinoplastia no Serviço de Otorrinolaringologia do Hospital de Clínicas - UFPR no ano de 2007. Resultados: Foram avaliados 79 pacientes; em 86% foi realizada a rinoplastia com técnica básica, em 6,4% e 7,6% foi realizado "delivery" e rinoplastia externa, respectivamente. Conclusão: Em nosso serviço realizamos rinoplastias com técnica básica na grande maioria dos pacientes.


Introduction: In the rhinoplasty, as in other surgeries, an adequate exposure of the manipulated structures is essential for a positive surgical result. Various techniques are used, and these may vary, mainly, because of the anatomical alterations found. Objective: To evaluate which are the most common surgical techniques and maneuver used in our service. Method: Retrospective analysis of the surgical descriptions of patients submitted to the rhinoplasty in the Otorhinolaryngology Department of the Clinical Hospital - UFPR in the year of 2007. Results: 79 patients were evaluated; in 86% of whom rhinoplasty with basic technique was performed, between 6,4% and 7,6% delivery and external rhinoplasty were used, respectively. Conclusion: In our service we performed basic technique rhinoplasty in the great majority of the patients.

Introdução: Parâmetros usados em cirurgias estéticas faciais atualmente são baseados em POWELL e HUMPHREYS (1). Estes autores formularam relações adequadas entre a face e o nariz, definindo os ângulos faciais. Objetivo: Comparar ângulos nasolabial, nasofrontal, nasomentoniano e nasofacial pré e pós-operatórios em pacientes submetidos à rinosseptoplastia. Método: Prospectivamente 37 pacientes, com média de idade de 30±11 anos, sendo 13 (36%) homens e 24 (64%) mulheres, submetidos à rinosseptoplastia. Os ângulos nasolabial, nasofrontal, nasomentoniano e nasofacial foram medidos e comparados, antes e depois da cirurgia, em fotografias de perfil. Resultados: Observou-se aumento médio de 8,6o no ângulo nasolabial, de 8,5o no nasofrontal e diminuição de 2,3o no ângulo nasofacial, sendo que todas as medidas apresentaram uma diferença estatisticamente significativa entre pré e pós-operatório. O ângulo nasomentoniano aumentou, em m??dia, 1,6o, diferença sem significância estatística. Conclusões: Os ângulos nasolabial e nasofrontal aumentam significativamente após a realização da rinosseptoplastia, enquanto o nasofacial diminui. Não foi observada alteração significativa no ângulo nasomentoniano com a realização da cirurgia.


Introduction: Parameters used in facial aesthetic surgeries are currently based on POWELL and HUMPHREYS (1). These authors have formulated suitable relations between the face and the nose, and defined the face angles. Objective: To compare pre and pos-operative nasolabial, nasofrontal, nasomental and nasofacial angles in patients submitted to rhinoseptoplasty. Method: Prospective analysis of 37 patients submitted to rhinoplasty, with an average of 30±11 years of age, being 13 (36%) men and 24 (64%) women. The nasolabial, nasofrontal, nasomental and nasofacial angles were measured and compared, before and after the surgery, in profile pictures. Results: An average increase of 8.6o in the nasolabial angle was observed, of 8.5o in the nasofrontal and reduction of 2.3o in the nasofacial angle, and all the measures presented a statistically significant difference between the pre and pos-operative period. The nasomental angle increased, on average, 1.6o, a difference without statistical importance. Conclusions: The nasolabial and nasofrontal angles increase significantly after rhinoseptoplasty, while the nasofacial angle diminishes. There was no significant alteration in the nasomental angle with the accomplishment of the surgery.

ABSTRACT

Se analizan las experiencias referidas por 100 pacientes, durante y después de las intervenciones quirúrgicas más frecuentes en cirugía estética, realizadas en forma ambulatoria, bajo anestesia local. Los procedimientos evaluados fueron: ritidoplastia cervicofacial (26%), rinoplastia(24%), mastoplastia de aumento (24%), blefaroplastia (16%) y expansión tisular (10%). El promedio de edad fue de 35,8 años. El sexo predominante fue el femenino (82%). El tiempo promedio por operación fue de 110 minutos. Solo el 3%de los pacientes debió consultar durante las 24 horas posteriores a la intervención. No hubo complicaciones inherentes al método. El 60%jerarquiza el método frente a la cirugía con internación. El 85%de los pacientes no encontraría inconvenientes con otra cirugía con el mismo método
